`Description: This is a passage about patient-portal-login. Lavonne Smith from Tomah Memorial discusses the new online Patient Portal, providing patients with their medical information at their fingertips. The MyChart portal is available through the hospital’s website or through a downloadable app.
I’m joined by Tomah Memorial Hospital information services director LaVon Smith who’s to fill us in on my chart patient portal that gives patients access to their personal healthcare information.
It’s been exciting last few months going through a change in our software and being able to offer online access to medical information if you’ve been a patient at Tomah Memorial Hospital since October 1st.
That’s my chart. You had to be a patient at Tomah Memorial Hospital since October first and received services. From there you can go to our website at tomahawk spittle.org, choose the patient portal button on the home page and sign up. You’ll use the register online link on the right hand side and fill in the demographics.
You’ll have access to your medical information when you need it. It is like the comfort of your own house. If a patient is logging in and trying to set up their their account and they have a little bit of a glitch or a question or they feel something in wrong, are there a number they can call or how would they get help with that?
We want to be there if you’re having some challenges and getting enrolled in it or experiencing an issue. We have assistance available and that can be called at three seven four six seven seven two.
You are giving those brochures to patients or those out available. In the hospital we are going to be rolling this out. This is our first step in making that announcement. The portal is all online. It’s web-based for the internet. Is there something for a person smartphone? Because those are big things for people with smartphones.
You will be able to go to Google store or the iPhone store online to look for my chart. Download that app and then you’ll be able to access your chart through the app so you got acident online. We have both options available.
What would be some of the benefits of being able to track your own chart stuff? Studies tell us that it’s key to be engaged about your health. So being access to the information, knowing your lab results, knowing the last time you had a chest x-ray and being able to access that online are key.
It’s something simple like knowing your blood pressure. What has your blood pressure been the last six months? Or when was the last time I saw the doctor at Tom burger? Knowing some of your medications would be important for a lot of patients. Having that at your fingertips either through your smartphone or through your computer at any time that you need it is excellent.
What an exciting new service that tol memorials offering their patients! You can find out more about how to access your my chart at Tomah Hospital.org. Slash my chart plus you can find this and past editions of health check 360 on our page.
